As stated last month, the prevalence of low back pain (LBP) is REALLY high! In fact, it’s the second most common cause of disability among adults in the United States (US) and a very common reason for lost days at work. The total cost of back pain in the US, including treatment and lost productivity, ranges between $100 billion to $200 billion a year! Is low back pain on the rise, staying the same, or lessening? Let’s take a look!

            In the past two decades, the use of health care services for chronic LBP (that means LBP > 3 months) has substantially increased. When reviewing studies reporting insurance claims information, researchers note a significant increase in the use of spinal injections, surgery, and narcotic prescriptions. There has been an increase in the use of spinal manipulation by chiropractors as well, along with increased physical therapy services and primary care physician driven non-narcotic prescriptions. In general, LBP sufferers who are chronic (vs. acute) are the group using most of these services and incurring the majority of costs. The reported utilization of the above mentioned services was only 3.9% in 1992 compared to 10.2% in 2006, just 11 years later. The question now becomes, why is this? Possible reasons for this increase health care use in chronic LBP sufferers may be: 1) There are simply more people suffering from chronic LBP; 2) More chronic LBP patients are deciding to seek care or treatment where previously they “just accepted and lived with it” and didn’t pursue treatment; or, 3) A combination of these factors. Regardless of which of the above three is most accurate, the most important issue is, what can we do to help chronic back pain sufferers?

            As we’ve discussed in the past, an anti-inflammatory diet, exercise within YOUR personal tolerance level, not smoking, getting enough sleep, and obtaining chiropractic adjustments every two weeks are well documented methods of “controlling” chronic LBP (as there really ISN’T a “cure” in many cases). You may be surprised to hear that maintenance care has good literature support for controlling chronic LBP. In the 8/15/11 issue of SPINE (Vol. 36, No. 18, pp1427-1437), two Medical Doctors (MDs) penned the article, “Does Maintained Spinal Manipulation Therapy for Chronic Nonspecific Low Back Pain Result in Better Long-Term Outcomes?” Here, they took 60 patients with chronic LBP (cLBP) and randomly assigned them into one of three groups: 1) 12 treatments of sham (fake) SMT (spinal manipulation) have over a one month period; 2) 12 treatments, over a one month period but no treatment for the following nine months; or 3) 12 treatments for one month AND then SMT every two weeks for the following nine months. To measure the differences between the three groups, they measured pain, disability, generic health status, and back-specific patient satisfaction at baseline, 1-, 4-, 7-, and 10-month time intervals. They found only the patients in the second and third groups experienced significantly lower pain and disability scores vs. the first group after the first month of treatments (at three times a week). BUT, only the third group showed more improvement at the 10-month evaluation. Also, by the tenth month, the pain and disability scores returned back to nearly the initial baseline/initial level in group two. The authors concluded that, “To obtain long-term benefit, this study suggests maintenance SM after the initial intensive manipulative therapy.” Other studies have reported fewer medical tests, lower costs, fewer doctor visits, less work absenteeism, and a higher quality of life when maintenance chiropractic visits are utilized. The question is, WHEN will insurance companies and general practitioners start RECOMMENDING chiropractic maintenance care for chronic LBP patients?

Low Back Pain Treatment

We have been providing low back pain treatment in San Francisco for over 20 years in the Financial District. We use a combination of chiropractic, deep tissue laser therapy, massage, exercise, and spinal decompression with the DRX9000.

Most of the time spinal decompression is not necessary, but when it is, it’s nice to have onsite.

The most common causes of low back pain are subluxations (misalignment’s) in the lumbar spine, sacrum and pelvis, bulging and herniated lumbar discs, spinal stenosis, degenerative disc disease, and Piriformis Syndrome. There are others but they are rare.

When it comes to low back pain treatment it can be a little tricky. Sometimes patients get better right away and sometimes it takes a while. It depends on the patients age, level of spinal degeneration, any complications, and the nature and severity of the underlying cause of the back pain (which can be difficult to identify).

So, we take all the information we have and formulate a treatment plan. A typical treatment plan at our SF back pain center is 10 visits or so over a 3-4 week period. We will use spinal adjustments, massage, stretching, ice, and home/work modifications to achieve favorable outcomes in the majority of cases. If low back pain or sciatica persists we may want to order an MRI of the lumbar spine to quantify the problem and rule out herniated or bulging disc pathology.

Our next step is spinal decompression with the DRX9000. This is typically 20 sessions over 6-8 weeks. The sessions are one hour long and include muscle stimulation and ice…laser if needed. The cost is $3500. Sure, it’s expensive but we have helped over 2000 patients with the DRX9000 and most are happy they did the treatment. It’s all about quality of life. Pinched Nerve in the Low Back?

We have been providing treatment for low back pain and sciatica in the SF Bay Area for over 20 years now. We use a combination of chiropractic adjustments, massage, motion extension traction, custom exercises, deep tissue laser therapy, and nonsurgical spinal decompression when indicated. A primary cause of low back pain and sciatica is a “Pinched Nerve” in the low back. Pinched nerves are generally caused by herniated or bulging discs in the lumbar spine, most commonly at L5-S1.

Nerve tissue is very sensitive to pressure. In fact, just the weight of a pencil erasure on a nerve can alter it’s ability to function by 60% and cause severe pain and dysfunction.

The sciatic nerve is the largest nerve in the body…it can be the diameter of a quarter at it’s greatest circumference. Bulging discs, herniated discs, spinal stenosis, disc degeneration, and various conditions like “Piriformis Syndrome” can put pressure on the sciatic nerve or nerve roots causing “pinched nerves”.

Typically, a patient will feel various combinations of low back pain, leg pain, numbness, tingling, aching, weakness, and muscle stiffness and spasms that become progressively worse over time. There can also be pinched nerves with no symptoms at all…for a while and then BOOM…when you least expect it lightning strikes. This is fairly common actually, and can be very alarming.

The solution for pinched nerves in the low back depends on the nature and severity of the problem. Conservative chiropractic, massage, laser, and ice are always the first thing we try. That is of course unless the patient is unable to handle it. In rare cases such as this we do laser and ice to try to reduce inflammation and then maybe some muscle stim and light massage.

Spinal decompression with the DRX9000 is also an option in the beginning as it is very safe and gentle. The DRX is designed and built to treat neurovascular compression syndromes such as bulging and herniated lumbar discs. However, the cost is greater than chiropractic as is the time commitment…so typically we try more conservative chiropractic first.

In any case, many chiropractors that have included disc decompression and deep tissue laser therapy into their protocols (such as us) have become “one stop shops” for pinched nerves due to herniated and bulging spinal discs.

Disc Bulges and Herniations: Most of Us Have Them

Disc Bulges and Disc Herniations are Common. That’s right. If we were to randomly select 100 people off the street and perform a diagnostic MRI to their lumbar spines, 60%+ would reveal disc bulges or disc herniations.

And most would not even have any symptoms or loss of function. But some would have severe low back pain and sciatica and be unable to function normally.

Heck, 80% or more of us will have a bout of severe back pain at least once in our life. And the #1 predisposing factor to chronic back pain is previous back pain. Back pain is epidemic.

Some experts believe discs are becoming injured all the time and the body is repairing these injuries itself. In fact, research suggests that it is best to wait and there is no real advantage to having QUICK low back surgery when it is determined a person has a herniated disc in the low back. It may heal on it’s own and there are nonsurgical treatments now to speed up the process or help initiate the process when the body is unable to.

BUT…just like the immune system is always battling germs and fighting disease (and some still get sick)…in some people the disc herniations do not repair themselves and they DO cause symptoms and loss of function….sometimes severe.  From my studies this occurs about 15% of the time and still amounts to a very large number of people. WHY?

Well…the discs get lodged and displaced in a certain way that puts pressure on the delicate spinal cord and nerve fibers and the body is unable to fix it. Then, the immune system attacks the area as if it were an enemy in a war over and over to prevent further harm (the inflammation process). The muscles go into spasms to protect the injured disc and restrict the motion that the disc needs to take in nutrients and waste products. The chain reaction can be devastating to some. The condition becomes chronic (over 90 days) and continues to degenerate until the nerve pressure can be removed.

Does this require surgery? Maybe…but maybe not!

There is a treatment called nonsurgical spinal decompression that is done on a machine that is designed and built to repair disc bulges and disc herniations in the low back and neck. The disc decompression machine we use in San Francisco is the DRX9000.

Just like a “respirator” helps the lungs to breath until they can do it on their own, a “spinal decompression machine” does the same thing for the spinal discs. The decompression process mimics the natural motion of the spine and helps to unlock the herniated discs and relocate them to a more normal position.

When Choosing a Spinal Decompression Doctor: Experience Matters!

Cervical Disk Herniations: Common Questions

Last month, we discussed the topic of neck pain arising from cervical disk herniations. The focus of this month’s Health Update is common questions that arise from patients suffering from cervical disk derangement.

1. “What can I do to help myself for my herniated disk in my neck?” The mnemonic device “PRICE” stands for Protect, Rest, Ice Compress, and Elevate is a good tool to use in the acute stage of many musculoskeletal conditions.

  • Protect your health by NOT placing yourself in an environment that is likely to harm you, such as playing sports or doing heavy yard work. That is, think about what you do BEFORE you do it and if sharp, radiating pain occurs, STOP and assess the importance of what you are doing. Use the concept, “…don’t pick at your cut.” This means if you want the injury to heal, don’t keep irritating it!
  • Rest is similar. Limit your activities to those that can be done without increasing symptoms, especially radiating pain.
  • Ice – The use of ice reduces swelling/inflammation, which reduces pain and promotes healing. Alternate it every 15-20 minutes (on/off/on/off/on) several times a day. You can also use contrast therapy (Ice/heat/ice/heat/ice) at 10/5/10/5/10 minute intervals to “pump” out the swelling.
  • Compress – The use of a collar worn backwards, if it’s more comfortable that way, can literally “take the load off.” the neck and disks. There are even inflatable collars which are pumped up with air to traction the neck. Other forms of traction will be discussed further.
  • Elevate – The concept of raising the ankle to the height of the heart so swelling can drain out of the ankle is the classic example of “elevation.” In the neck, the traction concept may apply once again.

 

2. “I don’t want to have surgery if I can help it. What can you do as a chiropractor to help me?” This is one of our primary goals, and in fact, the goal of ALL health care providers, even surgeons! Chiropractic offers anti-inflammatory measures: ice, herbal anti-inflammatory agents (ginger, turmeric, bioflavonoid, curcumin, bromelain, Rosemary extract, Boswellia Extract, and more), digestive enzymes taken between meals, muscle relaxant nutrients (valerian root, vitamin D, a B complex, chamomile, magnesium, and others) as well as other non-pharmaceutical options. Treatments consist of manual manipulation, mobilization, traction (for home and office), modalities such as deep tissue laser therapy and low-level laser, electrical stimulation, magnetic field, ultrasound, and others. We also offer nonsurgical cervical decompression with the DRX9000c. Most important is having a “coach” guide you through the stages of healing by first addressing the acute inflammatory stage (first 72 hrs), the proliferative or reparative phase (up to 6-8 weeks), followed by the remodeling phase (8 weeks to 1 or 2 years) and finally, the contraction phase (lifetime – includes the natural shortening of scar tissue). If manual traction reduces neck and arm pain, the use of home traction is very effective. Options include sitting over-the-door traction, laying down versions, and mobile traction collars (discussed previously). Exercises to stretch and strengthen the neck are also very important in reducing neck pain as well as preventing recurrences. If in spite of all the best efforts of this non-surgical care approach should ongoing neurological loss and relentless symptoms continue, we will coordinate care with physiatrists for possible injection therapy and pharmaceuticals, with neurology for further testing (such as EMG/NCV – a nerve test), and/or neuro- or orthopedic surgery – THE LAST RESORT!

How do you know if your spinal decompression is working?

We have been providing spinal decompression therapy in San Francisco for 8 years now. During this time we have performed over 20,000 decompression treatments to thousands of patients. We were the first to offer the DRX9000 in San Francisco.

The protocol for the DRX9000 is 20 sessions. The recommended treatment is 5x for 2 weeks, 3x for 2 weeks, and 2x for 2 weeks. When they conduct research this is what they do.

In reality, not all herniated or bulging disc patients can adhere to this treatment protocol due to family, work or school schedules. And honestly, sometimes we think the treatment might be too much for someone to handle. So…we treat each patient on an individual basis, sometime starting at only 2-3 times per week.

How do we know the 20 spinal decompression sessions will be enough?

We don’t…sometimes it takes more…but it rarely takes less. Sure, if you are a young adult (under 25 years old) and have a single level disc herniation, say at L5-S1, and an otherwise healthy spine, we may recommend 15 sessions.  Other than this scenario, we usually do 20-26 sessions.

In our experience it’s best to complete the program…even if you are feeling better after say 10 sessions. It’s kind of like when you take antibiotics…you feel better before the problem is fixed.

We often have patients that want to only do 10 sessions to see how they feel…but honestly…even if you are not feeling results, it does not mean the decompression therapy is not working. Some of our most dramatic results have happened at the end of treatment…in the last 5 sessions…and the results have lasted years.

And you should never stop after 10 sessions if you are feeling better…because you may risk a relapse.

What we recommend is deciding before you start that you are going to complete the program. Sure, we can split your payments up to make them more affordable…but don’t jeopardize your potential outcome by doing too little…that just does not make sense.

We will do it if you want…but we recommend you “stay the course”.  Because if you do the results will be more permanent. And we have the experience to know. We have seen patients that have done partial treatment get partial results. We we want this to be life changing and long lasting…and so should you!

Herniated Disc? What happens if you do nothing?

We have been providing treatment for herniated discs in San Francisco for about 20 years now. During this time we have learned much. About 7 years ago we added nonsurgical spinal decompression to our treatment protocols. A while later deep tissue laser.

So what happens if you are diagnosed with a herniated disc and you take no action?

It just depends.

Chances are if you have been diagnosed with a herniated disc you had an MRI…which is the “gold standard” diagnostic tool when it comes to confirming the presence of disc herniations or disc bulges.

And if you had an MRI you are most likely symptomatic…meaning you have symptoms such as sciatica from a lumbar disc herniation, or radiating arm pain from a cervical disc herniation.

So now your medical doctor tells you YES…you do in fact have a disc herniation in your lumbar spine at L5 and he wants to do surgery to remove it. WOW…what now?

Well…if it were me I certainly would not agree to low back surgery right off the bat. I would explore nonsurgical options such as nonsurgical spinal decompression with the DRX9000.

I would also consider major lifestyle modifications such as weight loss, giving up all bad habits such as smoking, drugs, drinking (if excessive) and anything else I was doing that was bad. I would seriously consider adopting a plant based diet such as the McDougall Program to reduce inflammation and help the disc to heal. I would go 100% clean and try to work it that way.
